Our aims and objectives
To provide a comprehensive range of health and social care services to clients over 65, enabling them to remain at home and preventing inappropriate admission to hospital or long term care.
What we do
We provide a comprehensive range of rehabilitative services provided by nurses, occupational therapists, physiotherapists, a dietician, a speech and language therapist, a podiatrist, a community psychiatrict nurse, rehabilitation support assistants, and social workers.
Additional Information
The team has two bases. Team 1 is at Govanhill workspace who cover the boundaries of the Gorbals Social Work Dept. Team 2 is at Castlemilk Social Work Dept and following their boundaries. The team is open to referrals from GP's, LHCC staff and Social Work Area Team staff.
